Photo ID, a list of any medicines you take and recent test results or hospital letters if you have them.
How do I change my appointment?
Let us know at least 3 hours before by phone or WhatsApp and we will find a new time.
What if it is an emergency?
The practice does not provide emergency care. In an emergency call 999 or go to your nearest A&E.
Do you do home visits?
